Care Management is a process to improve patient care by helping patients manage their overall health and conditions and reduce ER/Urgent care visits. This is done by helping you identify health goals and then developing a care plan to meet those goals. 
 

Examples:     If you feel your child would benefit from some guidance for behavioral issues related to ADHD or Anxiety
  If you feel your child would benefit from some nutritional counseling
  If you feel your child would benefit from some asthma education

 

Care Management is a covered service and is available to all patients.  

All visits are done here at our office or over the telephone with one of our care managers.

 

We have three care managers; one registered nurse and two medical social workers.

Britney Maike, RN

Britney received her RN degree from Lansing Community College in 2013. She began her nursing career as a General Medical Nurse at Munson Medical Center. Wishing to fulfill her nursing career in an outpatient atmosphere where relationships can be built and carried throughout many years, she became part of the GTCC team. Britney enjoys working with children and their parents to help with medical challenges, medication questions, care coordination, improving communication between parents and their child’s provider, and much more!

Kara Holcomb, LLMSW

Kara Holcomb is a Master’s level social worker. Kara received her BSW & MSW from Ferris State University.  Kara interned for over a year and a half at Adaptive Counseling & Case Management, through that experience she found that the mission and values of ACCM aligned with her own.  She has a passion for empowering individuals and helping them achieve their goals. Kara has worked in the health and wellness field for 18 years.  First working at Munson Medical Center and then working in the private sector Kara became interested in helping people navigate life when dealing with health and wellness.  Kara has a passion for helping individuals and families regarding depression, anxiety, family issues, loss & grief, transitions in life, chronic pain, and chronic illness.

Echo Dean, LMSW
Echo Dean has a Master’s degree in social work and has over 15 years of experience working with individuals from all age groups.  Echo received her BA degree in Family Life Education from Spring Arbor University and her MSW from Grand Valley State University.  In 2006 she completed the requirements to obtain her credentials as a Registered Play Therapist from the National Association of Play Therapy.  Echo’s social work career began at Child and Family Services as a foster care case manager and evolved into a role as therapist in the behavioral health department, working primarily with children but also with adults.  From there Echo moved into the field of medical social work, where she has been for the past five years.  From this experience, it became evident that many people with chronic illness need help to discover that they have the skills to improve their health but may need some direction and support to put together a plan to achieve their goals.  As a care manager working closely with the medical providers, Echo is able to provide this help and support.
