Financial Policy

 

Financial Policy:
The Grand Traverse Childrens Clinic welcomes you to our practice; office business hours are from 8 am to 4 pm.  We find it helpful to outline our financial policy to clarify expectations and responsibilities that each of us have.  We have business staff available to help you with questions regarding your bill, insurance claims, or other concerns; please feel free to call upon them for assistance.

Insurance Companies with which we participate:

  • Blue Cross Blue Shield (BCBS)

[Traditional, Blue Care Network, and Blue Choice]

  • Medicaid and Community Choice Medicaid

  • Priority Health
  • Tricare
  • United Health

Payment is expected at the time of service unless special arrangements have been made with our business staff.  The receipt we give you at the time of service is the only time a receipt can be given.  Please be sure to ask for one when you check out at the desk.  Insurance plans often have co-pays, which we must collect at the time of service.  Your insurance also may have a deductible to be met.  All newborns must be added to your insurance policy before their newborn care will be paid.  Please contact your employer to become familiar with your policy.  Please check with your insurance carrier regarding benefits for emergency care.  The physician on-call may recommend your child be checked in the emergency room, but he/she cannot advise you on insurance coverage.

Priority Health:
The co-pay is collected at the time of service.  You must present your card initially or if a change occurs.  A referral is needed prior to seeing another physician if they do not participate.   

Tricare Insurance:
Patients must present their current Tricare card.  Payment is expected at the time of service until you meet your deductible.  After your deductible is met, your co-pay is due on the date of service.

Blue Cross PPO:
We participate with BCBS PPO.  For maximum benefits, a referral is needed prior to seeing a non-participating physician.

 

State Medicaid and Community Choice:
The Medicaid HMO that we participate with is Community Choice.  We also take straight State Medicaid.  We need to see your card at every visit.  We will ask for payment if a Medicaid card is not presented.

Office Charges:
These prices may change at any time.  We provide them only as a guideline.

Office Call

$72 - $155

An office call fee will be charged for most visits

Physical Exam

$114 - $132

This fee applies to routine physical exam appointments except the 4-5 year pre-kindergarten exam.

New Patient Physical

$130 - $157

This fee is charged when establishing your child as a new patient in our practice.

Comprehensive
Office Call or Exam

$133 - $165

These visits are made for chronic or long-term problems.  More time is allotted than for a routine physical exam.

Most of these can be obtained free at the Health Department or Munson Medical Center Immunization Clinic for an administration fee.  Please keep your own record of these important immunizations.  All vaccines we administer will be recorded in the Michigan Immunization Registry.
